The Nurse Navigator uses advanced nursing processes in decision-making patient assessment data analysis development and implementation of methods and systems to improve the overall quality and efficiency of patient care for the designated department and population served. Under administrative oversight the Nurse Navigator will collaborate with the multidisciplinary teams at Carilion Clinic. The Nurse Navigator will provide patient education and help lead comprehensive conferences with the multidisciplinary teams. Provides direct patient care in the outpatient clinic and indirect patient care in inpatient and outpatient settings; oversees the delivery of patient care; and provides day to day management of programs to coordinate patient care as designated by the department. Coordinates the delivery of safe high quality patient care; identifies areas for study; and participates in and/or supports research. Works in compliance with all State Federal and other regulatory requirements. Serves as a resource and mentor to staff and team members. Contacts patients to instruct on appropriate follow up and coordinates care throughout.
Serve as the primary point of contact for patients and families throughout the care journey.
Provide education about diagnosis treatment options procedures and follow-up care in patient-friendly language.
Assess patient needs barriers to care and coordinate resources accordingly.
Facilitate scheduling of appointments tests and procedures to streamline care.
Promote adherence to treatment plans and medication regimens.
Conduct follow-up calls/visits to monitor progress and identify complications or concerns.
Act as a liaison between patients families physicians and the multidisciplinary care team.
Ensure communication and continuity of care between inpatient outpatient and community settings.
Participate in case conferences tumor boards or interdisciplinary rounds as appropriate.
Collaborate with social workers financial counselors and support services to address psychosocial and financial needs.
Educate patients and families about prevention disease management and healthy lifestyle choices.
Advocate for patients to ensure they receive timely appropriate and patient-centered care.
Provide education and support to staff regarding navigation processes and resources.
Collect track and report patient outcomes and navigation metrics.
Identify gaps in care delivery and contribute to process improvement initiatives.
Maintain knowledge of current clinical guidelines community resources and best practices.
Support program development outreach and community education as needed.
Document all patient encounters and navigation activities in the electronic medical record.
Ensure compliance with hospital state and federal regulations regarding patient care and confidentiality.
Participate in staff meetings training sessions and continuing education.
Education: Graduate of accredited school of nursing required.
Experience: Minimum of 3-5 years experience in Nursing required; 5 years experience preferred.
Licensure/Certifications/Registration: Current licensure as a Registered Nurse in the State of Virginia required.
Life support: AHA BLS-HCP required.
Other Minimum Qualifications: Proven ability to understand medical terminology problem solving and handle stressful situation with a professional manner. Good team dynamic skills in order to facilitate change and achievement of defined outcomes. Must be able to work with minimal amount of supervision. Demonstrates a positive demeanor excellent written and oral communication skills and a professional appearance and approach. Excellent organizational ability and initiative. Must have the ability to problem-solve and communicate with a wide variety of people in an effective manner. Able to work independently with minimal direction prioritize and meet timelines. Must exhibit skills to utilize automated computer systems including electronic medical records. Keyboarding skills required for automated computer systems including electronic medical records
