We are seeking a Registered Nurse to work in a combinedclinical and care management (care partner)role under the Support at Home program. This role suits an RN who enjoys providing in-home clinical care while also taking responsibility for care planning coordination and ongoing oversight of client outcomes.
You will act as the primary care partner for a portfolio of clients ensuring care remains clinically appropriate responsive and aligned with individual goals and funding arrangements.

Key Responsibilities
Clinical Care
- Deliver high-quality clinical nursing care in client homes in accordance with care plans clinical guidelines and organisational policies.
- Undertake comprehensive clinical assessments and contribute to the development and review of individualised care plans.
- Administer medications perform nursing interventions and monitor client responses to care.
- Identify clinical risks deterioration or changes in condition and escalate appropriately.
- Provide health education and support clients to maintain independence and wellbeing.
Care Management / Support at Home Responsibilities
- Act as the primary care partner for allocated clients under the Support at Home program.
- Coordinate services in line with assessed needs approved supports and client budgets.
- Work collaboratively with care workers allied health professionals GPs and external providers to ensure continuity of care.
- Monitor service delivery client outcomes and budget utilisation adjusting supports as needs change.
- Support clients and families to understand their care plan services and responsibilities under Support at Home.
- Maintain accurate clinical and care management documentation to meet regulatory funding and quality requirements.
- Contribute to clinical governance continuous improvement and quality assurance activities.
