The Opportunity
As a Home Health Physical Therapist youll take ownership of patient outcomes by delivering individualized goal-driven therapy in a home-based setting. Youll combine clinical expertise with creativity adaptability and compassionhelping patients regain function on their terms.

How Youll Make an Impact
Conduct in-home evaluations to assess mobility strength balance and functional limitations
Create and adjust personalized therapy programs aligned with physician orders and patient goals
Provide hands-on treatment focused on restoring independence and preventing re-injury
Collaborate with RNs OTs SLPs and physicians to ensure coordinated care
Educate patients and caregivers on safe mobility exercises and fall prevention
Supervise and support therapy assistants as needed
Complete accurate and timely documentation in compliance with agency and regulatory standards
Serve as a trusted therapy resource within the communities you support

What Were Looking For
Degree from an APTA-accredited Physical Therapy program
Active Physical Therapist license in the state of Michigan
Minimum of 1 year of clinical experience (home health preferred not required)
Comfort working independently in the field with strong clinical judgment
Reliable transportation valid drivers license and auto insurance
Ability to travel within the service area (up to 50%)
Physical ability to assist lift and transfer patients (up to 50100 lbs)
