At Elara Caring were redefining what a full-time therapy career can look like. This Physical Therapist role offers a flexible four-day workweek giving you the ability to design a schedule that supports your life while continuing to deliver meaningful one-on-one care to patients in their homes.

The Role

As a Home Health Physical Therapist you will provide individualized in-home therapy services that help patients regain mobility independence and confidence. Youll manage a full-time caseload across four workdays allowing for improved work-life balance without compromising quality or outcomes.

What Youll Do

  • Conduct in-home physical therapy evaluations and functional assessments

  • Develop and implement personalized treatment plans aligned with patient goals and physician orders

  • Provide hands-on therapy focused on strength balance gait and mobility

  • Modify care plans based on patient progress and changing clinical needs

  • Collaborate with physicians nurses and interdisciplinary team members

  • Educate patients and caregivers on safe mobility exercises and fall prevention

  • Complete timely accurate clinical documentation in accordance with agency standards

  • Serve as a clinical resource within the communities you support

What You Bring

  • Graduate of an APTA-accredited Physical Therapy program

  • Active unrestricted Physical Therapist license in the state of practice

  • Minimum of one year of clinical experience (home health experience preferred)

  • Strong clinical judgment and comfort working independently

  • Reliable transportation valid drivers license and current auto insurance

  • Ability to travel within the assigned territory

  • Physical ability to lift transfer and assist patients (up to 50100 lbs)
