LPN MDS Nurse

Rate: $30 - $33/hr. (depending on experience)

At Seneca Health Care Center were seeking a detail-oriented and compassionateLicensed Practical Nurse (LPN)to join our team as anMDS Nurse. In this role youll help ensure residents receive the right care at the right timeby using your clinical knowledge and documentation skills to support accurate assessments and individualized plans.

Your Role Includes:

• Completing and coordinating timely MDS assessments and submissions

• Collaborating with the interdisciplinary team on resident care plans

• Ensuring documentation meets regulatory and quality standards

• Tracking resident changes and updating care plans accordingly

• Supporting audit readiness case-mix accuracy and survey prep

What Were Looking For:

• Valid NYS LPN license

• Experience with MDS (preferred) or willingness to learn

• Strong attention to detail and clinical judgment

• Excellent communication and teamwork skills

• Prior experience in skilled nursing or long-term care a plus
