Position Summary
The organization is seeking a Family Nurse Practitioner to join its expanding outpatient primary care team in Naugatuck Connecticut.

Essential Responsibilities
Provide comprehensive primary care services to patients across the lifespan
Diagnose and manage acute and chronic medical conditions
Conduct preventive care visits annual physicals and wellness screenings
Prescribe medications and develop evidence-based treatment plans
Collaborate with physicians and multidisciplinary care teams
Maintain accurate and timely EMR documentation
Educate patients on health maintenance and disease prevention
Qualifications
Education: Masters or Doctoral degree from an accredited FNP program
Certification: National certification as a Family Nurse Practitioner
Licensure: Eligible for Connecticut APRN licensure
Experience: Prior primary care experience preferred; new graduates welcome
