The Innovative Healthcare Collaborative of Indiana (IHCI) is a joint venture between Community Health Network and Deaconess Health System. Its goal is to support our sponsors and partners in their strategic evolution to positively impact and improve the healthcare delivery system. As a member of the team you will have the opportunity to make a profound impact on the lives of people living with complex and/or chronic disease. This position is primarily remote but there is potential for some physician office-based duties. Patient coordination will be telephonic. The Outreach Specialist will evaluate patients who have stratified into potential care management programs or have recently discharged from an emergency department or hospital stay. The role will work closely with an IHCI RN Care Advisor to support and improve IHCI care management processes. The specialist will introduce care management program options to patients assess their willingness to participate and if the patient approves schedule initial assessment appointments with an IHCI RN care advisor. Specialist will assist with patient care gap identification and closure through telephonic patient contact EMR evaluation and payer portal assessments. The specialist will assist patients with scheduling any necessary exam or test to close gaps in care. Additionally he/she will use critical assessment skills to coordinate care and schedule any necessary post-discharge follow up appointments.
While many of our positions offer the flexibility of a primarily remote work environment some roles operate under a hybrid model requiring a blend of telephonic and in-person interactions to best support our operational needs and foster team collaboration. Patient interaction may occur in the physician office setting on the phone at their home or in the hospital setting - essentially however and wherever the patient needs your assistance to improve their health better understand their illness and coordinate their care. Specific work arrangements will be detailed for each individual role
Exceptional Skills and Qualifications
Ability to navigate EMRs
Ability to utilize Microsoft Office Suite
Ability to manage projects and assignments from start to completion
Ability to use customer service skills to provide an exceptional experience
Ability to collaborate and build effective relationships
Ability to maintain confidentiality and understand HIPAA regulations
Ability to improve processes
Ability to work under pressure
Ability to communicate complex/ technical information effectively both orally and in writing
Ability to effectively maintain databases
Ability to self-motivate
Ability to travel between locations
Ability to be detailed oriented and organized
High school diploma or GED equivalent. (Required)
2 year / Associate Degree. (Preferred)
3 years: Experience with medical/business computers applications in provider office or healthcare setting. (Required)
