Major Unusual Incident Investigator
A typical day in the life of a Major Unusual Incident (MUI) Investigatorinvolves being out in the field meeting with individuals who have been involved in a Major Unusual Incident which are serious events involving individuals with developmental disabilities such as allegations of abuse neglect exploitation or unexplained injuries that require formal investigation and reporting to state authorities. You will regularly interact with internal and external Agency departments members of the community family members guardians direct care staff providers law enforcement and court staff to name a few. You will use an Agency-provided phone and laptop to facilitate and attend phone calls Zoom and Teams meetings and/or meetings in person. Your goal is to ensure that a complete administrative investigation is conducted and preventative measures are implemented to ensure the health and welfare of the individual(s) served. You will complete a comprehensive investigation summary for each assigned case.
This is an outwardly facing position which would necessitate frequent meetings in individuals homes.
Job Duties (Summary):
  • Conduct thorough investigations of incidents involving individuals with developmental disabilities including interviewing witnesses reviewing physical evidence and collecting reports from relevant sources
  • Collaborate with local law enforcement agencies and the Department of Jobs & Family Services on joint investigations as needed
  • Assess and respond to incoming incident reports and Department of Developmental Disabilities (DODD) Hotline complaints to determine appropriate action
  • Prepare comprehensive investigative reports documenting findings and provide recommendations to prevent future incidents
  • Ensure all investigations comply with Ohio Administrative Code regulations and statutory requirements including proper documentation and timely follow-up
  • Travel to community locations and work sites throughout the county to conduct on-site investigations
  • Maintain accurate records of all investigations reviews and services provided while ensuring strict confidentiality of sensitive information
  • Participate in professional development opportunities including meetings seminars and conferences to stay current on best practices in incident investigation
  • Serve on office agency local and state committees as assigned to support continuous quality improvement.

Employees are expected to travel within the county to meet with individuals and attend in-office meetings as needed. The flexibility to work from home is available when job duties allow.
