Job Summary

Our dedicated HIS team will collaborate closely with our trusted HIS vendor (Epic) and internal subject matter experts to develop a fully functional system that will empower us to deliver exceptional care and services.

The primary purpose of the Manager Epic Community Partnerships is to support the the strategic adoption of health information systems with responsibility for leading systems integration across Oslers external stakeholders as external stakeholder partnerships and their HIS requirements evolve. This role is responsible for developing strengthening and sustaining strategic relationships between the hospital and external partners with a focus on primary care providers family health teams and community organizations that support patients across the continuum of care. This role enhances coordination of care supports integrated patient pathways and advances population health initiatives. A key component of this role includes supporting community partners in the adoption and optimization of Epic Link to improve information sharing referral workflows and care coordination between the hospital and external providers.

Accountabilities:

  • Build nurture and maintain strong partnerships with primary care physicians family health teams home and community care agencies and social service providers
  • Act as the primary liaison for primary care and community stakeholders promoting seamless coordination that benefits patients
  • Support onboarding engagement and training of community partners using Epic Link to access hospital information clinical results and patient updates
  • Act as the primary liaison and vendor representative for all Connect sites ensuring strong communication expectation-setting and relationship management
  • Lead community engagement related to Epic Link adoption including outreach demonstrations onboarding and user support
  • Work with Information Services clinical departments and the Epic implementation team to resolve issues gather feedback and improve workflows for external providers
  • Promote the use of Epic Link to enhance communication referrals transitions of care and shared care planning with community partners
  • Coordinate and deliver role-specific training and onboarding for Connect site users ensuring they understand system functionality workflows and policy expectations
  • Provide post-go-live stabilization support and continuous improvement guidance to ensure sustained success
  • Lead and coordinate the onboarding of community practices onto the organizations Epic instance ensuring smooth integration of workflows data and operational processes
  • Oversee configuration build and testing activities to support each sites unique needs while maintaining compliance with enterprise standards
  • Ensure appropriate user provisioning security model alignment and system access for Connect partners
  • Lead integrated care initiatives that strengthen coordination between the hospital primary care and community services
  • Oversee projects that improve patient transitions chronic disease management pathways preventative care collaborations and shared clinical programs
  • Collaborate internally to ensure community partnership activities align with patient needs and hospital strategic priorities
  • Develop and maintain a comprehensive community Connect strategy and business plan ensuring alignment with organizational goals strategic growth priorities and partnership opportunities
  • Identify potential community partners (e.g. PCP clinics specialty practices small hospitals) and assess their readiness for Epic hosting
  • Establish service-level agreements contract expectations and performance standards for participating community practices
  • Conduct assessments that identify gaps in primary care connectivity digital readiness community support capacity and patient access
  • Track Epic Link utilization metrics and prepare reports for leadership and community partners
  • Monitor system changes population needs and emerging trends in integrated care planning
  • Monitor adherence to contractual obligations including uptime maintenance windows and cost-sharing agreements
  • Establish metrics and dashboards to track utilization performance patient experience impacts and cost efficiencies across Connect sites
  • Conduct regular reviews and present program outcomes to leadership and stakeholders
  • Coordinate primary care and community roundtables Epic Link information sessions and joint planning meetings
  • Develop communication materials such as user guides partnership updates briefing notes and presentations
  • Lead cross-functional teams focused on integrated care community collaboration and Epic Link optimization
  • Provide guidance to staff supporting outreach transitions of care and partnership programs
  • Ensure activities support health equity access to care and patient-centred outcomes
  • Ensure all Connect programs adhere to regulatory privacy security and organizational compliance standards
  • Manage onboarding workflows for Epic Link providers
  • Develop policies procedures and best practices for community partnership engagement and Epic Link utilization
  • Provide excellent leadership skills with strong ability to coach and mentor team members through each phase of the HIS project
  • Advise and coach team members by providing guidance on project issues key success factors of the project and lessons learned

Qualifications :

  • Bachelors degree in nursing health care administration or related degree required
  • Masters degree (MHA MPH MSW MBA or related) preferred
  • Epic certification in Ambulatory Link Health Planet Link required
  • Minimum of five years experience implementing and operating modern HIS platforms
  • Demonstrated experience in IT-enabled projects involving complex change management
  • Experience working with primary care providers community agencies or health system transformation initiatives
  • Experience with Epic systems
  • Strong relationship-building facilitation and communication skills
  • Understanding of primary care models integrated care digital health tools and social determinants of health
  • Project management problem-solving and multi-stakeholder coordination
  • Proficiency with Microsoft Office project platforms and data tracking tools

Hours: Currently days (subject to change in accordance with operational requirements)

Salary:
Minimum: $118716.00
Maximum: $148395.00
