Department: Home Care/ Liaison Thome PACE
What Youll Do in This Role:
The LPN Home Care Coordinator Facility Liaison provides direct and indirect nursing care to PACE participants ensuring their health needs are met across home and facility settings. Under the supervision of the Community RN Supervisor or designee this role applies advanced nursing skills and clinical judgment to manage complex situations coordinate care monitor participant progress and address emerging health concerns. Acting as a key link between participants families and healthcare providers the Home Care Coordinator collaborates closely with physicians nurses and interdisciplinary team members to deliver high-quality participant-centered care.
Assessment & Care Planning
- Collect participant data and complete required forms per PACE standards.
- Identify and prioritize participant needs and problems.
- Conduct facility assessments of prospective participants and complete annual assessments.
- Develop and revise care plans with input from participants families facilities and the interdisciplinary team (IDT).
- Make referrals to support services as needed.
Care Implementation & Management
- Provide direct nursing care and manage medications for participants in facility settings.
- Conduct regular facility visits to ensure care plans are followed.
- Document all care in the EMR and communicate changes to the care team.
- Coordinate durable medical equipment (DME) and facilitate respite discharges.
- Meet monthly with Directors of Nursing and facility managers to coordinate care.
Leadership & Quality
- Attend clinic meetings and delegate nursing care as appropriate.
- Participate in Quality Improvement (QI) initiatives and maintain safe work practices.
What Youll Need:
- Graduate of an accredited School of Nursing.
- Licensed Practical Nurse (LPN) in the State of Michigan.
- Must have reliable transportation and possess a valid and current drivers license.
- Must be CPR certified or willing to obtain
Desired Qualifications:
- Minimum of three (3) years of experience as a practicing nurse in a health-related area.
- At least one (1) year of experience working with the frail and elderly particularly in a homecare or community setting.
- Experience in managing or coordinating patient care especially for individuals with complex health needs.
- Knowledge of or experience in working within a PACE (Program of All-Inclusive Care for the Elderly) program or similar
- Elderly care models is a plus.
- Ability to effectively communicate with patients families and interdisciplinary teams.
- Strong critical thinking and problem-solving skills with the ability to work independently and manage complex clinical situations.
