Job Summary
Job Title: Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN)

Position Summary
The Licensed Practical Nurse (LPN/LVN) provides direct nursing care to individuals with developmental and/or long-term care needs under the supervision of a Registered Nurse (RN). This role involves administering medications performing clinical procedures monitoring patient conditions and supporting daily living activities in a residential care setting.
Key Responsibilities
Administer medications and injections as prescribed
Perform tube feedings hang IVs and provide colostomy care
Collect blood samples (if venipuncture certified)
Monitor patient condition and report changes to RN or physician
Take and document vital signs (temperature pulse respiration blood pressure)
Obtain specimens for laboratory analysis
Prepare and apply simple dressings
Assist with bathing feeding hygiene and activities of daily living (ADLs)
Support residential care duties including bed-making linen changes lifting and repositioning patients
Maintain patient safety and ensure continuity of care at all times
Scheduling Details
Night shift: 4x10s (10:45 PM 9:15 AM)
Days off: Sunday Monday Tuesday
Floating may be required to other residential locations based on staffing needs
Compliance & Facility Requirements
This is a mandated care environment
In the event of staffing shortages the contractor may be required to remain on duty until relief arrives in accordance with state regulations
SCR and SEL clearance required prior to start (processing may take 24 weeks)
Licensure & Requirements
Active New York State LPN license (must be in hand)
Ability to work mandated shifts and float as required
Experience in long-term care developmental disabilities or residential settings preferred
