ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
- Work closely with the physicians and advance practice providers to deliver the best care for our patients in an outpatient setting.
- Set-up medical equipment and perform related tests as applicable administer injections and perform routine specimen collection and point of care tests.
- Responsible for vaccine administration as directed.
- Conduct telephone and EMR triage for patient health complaints using the electronic triage assessment tool.
- Follow up with patients via phone after emergency department and hospital visits.
- Document nursing triage notes into the EHR system Epic for clear and concise documentation.
- Assess identify and close patient care gaps. Identify patient needs to establish plan of action or next steps.
- All other duties as assigned.
QUALIFICATIONS:

Education Licensures & Certifications
- Required: Bachelors degree in Nursing
- Required: Active and unrestricted registered nurse license in the state of Ohio
- Preferred: Experience in telephonic nursing triage
- Preferred: Experience in population health addition to providing basic triage services this position will focus on population health management through direct patient outreach.
Knowledge Skills & Abilities
- Exceptional active listening communication and documentation skills
- Excellent customer service skills and mind-set
- Self-disciplined energetic passionate innovated and team spirited.
