About the role
As a Home Support Partner (Case Manager) youll help older people in Greater Sydney live their best life. Youll assess needs create care plans and support clients with complex requirements. Working closely with community care workers youll ensure services align with the consumer-directed care model. Youll also promote our services at local expos and community events.

Skills and Experience
Experience in case management within ageing services or a minimum Certificate IV in Case Management.
Ability to work in a person-centred holistic way to achieve positive outcomes for clients.
Strong interpersonal and customer service skills with the ability to communicate effectively with people from diverse backgrounds.
A collaborative team player who can balance competing priorities while maintaining self-care.
Excellent organisational skills and confidence to represent the service at community events.
Current drivers licence is essential.
