About the role

As a Home Support Partner (Case Manager) youll help older people in Greater Sydney live their best life. Youll assess needs create care plans and support clients with complex requirements. Working closely with community care workers youll ensure services align with the consumer-directed care model. Youll also promote our services at local expos and community events.

Key Responsibilities
  • Support clients under the Commonwealth Home Support Programme (CHSP) and Home Care Package (HCP) by completing assessments and creating personalised care plans.
  • Use a relationship-focused approach to identify each persons goals and abilities.
  • Build and maintain strong networks with agencies health professionals and community services including ACATs GPs social workers and culturally specific organisations.
  • Provide person-centred case management in collaboration with clients families and carers.
  • Make it easier for people with disability to access services that match their goals working with funded providers and other agencies.

Skills and Experience

  • Experience in case management within ageing services or a minimum Certificate IV in Case Management.
  • Ability to work in a person-centred holistic way to achieve positive outcomes for clients.
  • Strong interpersonal and customer service skills with the ability to communicate effectively with people from diverse backgrounds.
  • A collaborative team player who can balance competing priorities while maintaining self-care.
  • Excellent organisational skills and confidence to represent the service at community events.
  • Current drivers licence is essential.
