This is a growing program which is so needed in our communities. The program serves our homebound patients giving them access to palliative care in the comfort of their homes. 1-3 years of APP experience preferredPalliative Care Certification preferredWeekly Monday - Friday scheduleNo call requirementOutpatient palliative care in patients homesVisit patients in assigned territoryWork collaboratively with Social Workers and House Calls Physicians & APPsCompetitive compensation benefits CME and occurrence-based malpractice insuranceAnnual Salary: $110K - $120KQualificationsREQUIRED QUALIFICATIONS:Education/TrainingGraduate of an accredited school of nursing and nurse practitioner programMasters degree in Nursing or Physician Assistant StudiesCurrent Illinois license in good standingCertified as a nurse practitioner or physician assistant by an approved certifying authority Illinois Controlled Substance DEA certificationCurrent CPR certification
