Position Snapshot
- Status: PRN / As Needed
- Pay: $50 per visit
- Pay Frequency: Weekly
- Location: Amarillo TX (Home Health)
- Schedule: Flexible and nurse-driven

What Youll Do
- Serve as the first clinical point of contact for new patients and families
- Complete pre-admission assessments evaluating medical needs safety and home environment
- Determine admission eligibility based on physician orders and regulatory standards
- Develop and update Plans of Care reflecting patient needs and goals
- Obtain start-of-care orders and all required intake documentation
- Coordinate care with physicians and interdisciplinary team members
- Provide patient and caregiver education that supports safe successful care at home
- Deliver skilled nursing care aligned with BrightStar Care standards and Texas regulations

What Were Looking For
- Active Registered Nurse (RN) license in the State of Texas
- Minimum of two (2) years of RN experience
- Strong assessment organization and communication skills
- Comfort working independently with the backing of a supportive local team
