Weekly Gross Pay: $2286
W2 Rate: $26/hour
Weekly Stipend: $1246
Position Summary
We are seeking an experienced Physical Therapist (PT) for a 13-week contract assignment in a Skilled Nursing Facility / Long-Term Care setting in Montour Falls NY. The ideal candidate will deliver high-quality rehabilitative care to residents while managing a full caseload independently in a post-acute care environment.
Key Responsibilities
Evaluate residents physical conditions functional limitations and rehabilitation needs
Develop and implement individualized treatment plans based on clinical assessments and physician orders
Provide therapeutic exercises gait training manual therapy and functional mobility training
Monitor patient progress and adjust treatment plans to optimize outcomes
Document evaluations daily treatments progress notes and discharge summaries accurately and timely
Collaborate with nursing physicians and interdisciplinary care teams to support resident goals
Educate residents and caregivers on treatment plans mobility strategies and safety precautions
Maintain compliance with regulatory standards safety guidelines and infection control protocols
Required Qualifications
Active New York State Physical Therapist (PT) License (required)
Minimum 2 years of Physical Therapy experience
Ability to work independently and manage a full caseload
Strong clinical judgment communication and documentation skills
Preferred Qualifications
Experience in Skilled Nursing Facility (SNF) or Long-Term Care settings
Previous travel experience (preferred not required)
Experience in post-acute or long-term care environments
