Position Overview
The Case Coordinator will determine program eligibility and assess the need for care at home or in a personal care home for the elderly or infirm adult. He/she will coordinate the delivery of a broad range of professional and non-professional services based on the Case Coordinator Guidelines. Acts as a liaison and maintains communication with agencies and facilities involved with the client group.

Experience
- Two years directly related community experience preferred.
- Previous experience as a Case Coordinator in Community Health Services preferred.
- Experience with EFT implementation considered as asset.

Expérience
- De préférence deux ans dexpérience dans la communauté directement en lien avec le poste.
- De préférence expérience antérieure à titre de coordonnateur/coordonnatrice de cas dans des services de santé communautaires.
- Une expérience de la mise en œuvre dun programme dETP est considérée comme un atout.

Education (Degree/Diploma/Certificate)
- BSW OT PT BSc PN RPN BN RN SLP RD or related health/human service degree required.
Éducation (diplômes et certificats)
- B. Serv. soc. ergothérapie physiothérapie B. Sc. IPS inf. aux. aut. B. Sc. inf. inf. aut. orthophonie diététique ou diplôme connexe en sciences de la santé ou en services sociaux requis.
Certification/Licensure/Registration
- Valid drivers license and a vehicle.
- Licensure/registration as per professional designation required.
- If successful applicant is a Registered Dietitian must be a graduate of a recognized dietetic internship program accredited by Dietitians of Canada.
Accréditation/permis dexercer/inscription
- Posséder un permis de conduire valide et une voiture.
- Titulaire dun permis ou dune autorisation dexercice dans sa profession.
- Si la personne retenue est diététiste elle doit être diplômée dun programme de stage en diététique reconnu par Les diététistes du Canada.
Qualifications and Skills
- Must be able to speak read and write French at the predetermined linguistic level.
- Knowledge and experience with computers required.
- Effective oral and written communication skills.

- Connaissance pratique de linformatique requis.
