A healthcare facility located near Fort Worth TX is looking for a Hospice Community Liaison to join their team. Details are below!
Salary: $60000 - $125000/annually
Hospice Community Liaison Opportunity:
- Proven history of driving hospital-based hospice referral sources located in Fort Worth.
- Hospice Community Liaison must have existing strong relationships with hospital case managers discharge planners social workers oncology units ICU care teams and ED staff that have told the rep that if they moved to another hospice they would continue referring to them
- Ability to navigate hospital systems (Texas Health Baylor Scott & White Medical City JPS).
- Strong at creating daily rounding patterns building trust with care coordinators and being physically present.
Hospice Community Liaison Qualifications:
- Have multiple hospital accounts routinely referring at least 1-2 patients a month
- Ability to open new hospital accounts
- Must have a proven track record of 6 referrals/admissions a month from key accounts that arent dependent on current/past hospices MD or relationships.
- Hospice Community Liaison must be able to obtain GIP contracts with at least all their top referring hospital accounts
