- Key responsibilities: Provide direct patient care in a Med Surg Tele unit including comprehensive telemetry monitoring medication administration (oral IV and infusions as assigned) wound and IV therapy post-op and medical/surgical patient management and rapid response when patient status changes. Perform ongoing assessments develop and update individualized care plans coordinate with physicians charge nurses pharmacists and case managers and facilitate effective discharge planning with patient and family education. Accurately document in the electronic health record ensure patient safety through evidence-based practices and participate in rounds and care coordination meetings.
