Your Responsibilities
- Deliver personalized high-quality care to seniors in their homes assisted living hospice and nursing facilities.
- Conduct comprehensive assessments and develop tailored care plans for older adults.
- Manage chronic conditions with skill and empathy for patients and families.
- Collaborate with cross-functional partnersincluding palliative psych and neuroto deliver coordinated whole-person care.
- Build trusted relationships with residents caregivers and facility partners to enhance engagement and referrals.
- Maintain consistency in documentation follow-through and patient communication.
What Were Looking For
- Active unencumbered Physician Assistant license in the state you will be practicing.
- Minimum of 12 years of clinical experience (geriatrics or primary care preferred).
- Leadership potential or experience: Ideally youve managed a panel successfully and are ready to grow into leadership roles.
- Experience in home-based care SNF or ALF settings strongly preferred.
- Compassionate patient-centered approach and strong clinical judgment.
- Valid drivers license and reliable transportation.

Pay Range for Posted Region
