PSYCHIATRIC SERVICES CARE TEAM NURSE (LPN/RN)

Location: West Salem Clinic 1233 Edgewater Street NW Salem OR 97304

Job Status: Full-Time Monday - Friday 8:00 am 5:00 pm

We are hiring an LPN/RN to provide practice management support direct care and work as an extended provider to support patients in between visits with their provider.

YOUR ROLE:

As our Psychiatric Services Care Team Nurse you willwork in collaboration with other care team members to address patients psychiatric needs and works closely with the Primary Care Provider the Psychiatric Care Provider the Psychiatric Intake Coordinator and the patients care will oversee the respective providers assigned patient panel by directing and facilitating care to support the patient and incorporating the nursing process: assessment planning intervention and evaluation.

SPECIFIC DUTIES:

  • Demonstrates cognitive ability to accurately observe analyze and interpret general and technical data both verbal and written and to accurately communicate this information. Recognizes abnormal and critical clinical findings and takes appropriate action including notifying the provider. Takes responsibility for the facilitation and coordination of medication management as prescribed by the Psychiatric team. Coordinate services involving the PCP as needed for example scheduling follow-up appointments to ensure continuity of care.

  • Participates in the ongoing management of patients with mental health diagnoses as designated by and in consultation with the Psychiatric Provider. Communicate with patients in person and over the telephone regarding issues concerning the management of their mental health diagnosis coordination of treatment plans/changes as well as coordination of services. Includes follow-up with the patient when indicated. Providers care via telephone or in-person appointments for reassurance and to monitor for side effects until future appointments with their prescriber for example starting a new medication.

  • Demonstrates strong critical thinking skills able to maintain composure during challenging events able to de-escalate clients and can problem solve effectively. Is a resource to the support team proactively responds to clients in crisis in the lobby if a Behaviorist team member is unavailable deescalating the situation and making a safety plan if needed. Communicates effectively in a professional manner with clients staff and providers updating the team of a change in client presentation.

  • Huddles throughout the day with psych providers and Psychiatric Services Care Team Medical Assistants identifies deficiencies in workflow and proactively collaborates with the entire care team to problem solve. Performs point-of-care testing such as collecting lipids and A1C lab values including entering into NextGen. Performs ECG testing when needed.

  • Manages the Psychiatric Care Team Inbox making telephone calls to patients or others as needed. Directs and delegates to the care team when necessary.

QUALIFICATIONS:

  • LPN or RN License issued by the Oregon State Board of Nursing. Experience in Behavioral Health is a plus.
  • 2 years Nursing experience required.
  • Experience working with Psychiatric Services/patients is required.
  • Experience or demonstrated ability to coordinate and integrate services and work as a team member.
  • Demonstrated ability to perform the competencies required of a Qualified Mental Health Professional/LPN/RN according to the Oregon Administrative Rules for Mental Health Programs.
