Department Overview

The Home Care and Hospice department at Childrens provides care and support for patients who live in the Twin Cities seven-county metropolitan area. The team is made up of registered nurses clinical social workers child life specialists chaplains and intake coordinator who work collaboratively to provide care in the patients home. Services provided include home care and home care pharmacy hospice care palliative care perinatal hospice program and Childrens Institute for Palliative Care.


Position Summary

Coordinates and provides family-centered nursing care to infants children and adolescents in the home setting. Accountable for professional practice including independent nursing functions and delegated medical functions performed in collaboration with other health members. Responsible for continuous review of all aspects of every patient on his/her caseload to include: appropriate utilization of services; ensuring continued skilled need; monitoring of homebound status; review of documentation in the medical record; maintenance of interdisciplinary communication and discharge planning.

Education/Certification:
*Current Minnesota RN License required.
*BSN preferred.
*Current BLS Certification required.
*Pediatric experience preferred.
*Home care experience preferred.
*Infusion experience preferred.
*Venipuncture skills preferred.

Requirements:
*Must have a working automobile. Mileage reimbursed based on federal rate.
*Must possess a valid drivers license (must meet minimum criteria of Childrens Minnesota Fleet Safety Management policy).
*Clean driving record.
*Must possess and provide verification of current automobile insurance coverage.

The posted salary represents a market competitive range based on salary survey benchmark data for similar roles in the local or national market. When determining individual pay rates we carefully consider a wide range of factors including but not limited to market indicators for the specific role the skills education training credentials and experience of the candidate internal equity and organizational needs.

In addition to your salary this position may be eligible for medical dental vision retirement and other fringe benefits. Positions that require night weekend or on-call work may be eligible for shift differentials or premium pay.
