Aging Waiver Support Coordinator --
Trimed HealthCare invites you to join our team of compassionate care givers as a Aging Waiver Support Coordinator and earn a competitive salary. We offer you flexible schedules with opportunity to advance - and a chance to support the consumer within the comfort of their homes and community - and do the the work you are passionate about. Requires at least 1 year case management experience / supports coordination in the PA State Home and Community Based Waiver Services. Bachelor Degree level applicant. Experience in SAMS and HCSIS.
