Major Duties and Responsibilities

Patient Coordination

Plays an active role in patient intake reviewing medical records for the appropriateness of home health care services. Provides clinical direction to field staff as defined by their disciplines scope of practice.

  1. Assists with the preparation or revision of the patients plan of care.
  2. Supports the administrative functions of the Nursing Department.
  3. Assists with supervising and reviewing nursing staff.
  4. Facilitates intake through the implementation of direct care services.
  5. Receives calls from patients and follows up on issues from notification to resolution.
  6. Conducts quality assurance telephone calls to patients or caregivers to assess their satisfaction.
  7. Assists in client incident investigations and resolves patient and/or caregiver complaints.
  8. Supports the Director of Nursing in determining staffing levels.
  9. Reviews patient intake information and assists in managing patient data and medical records.
  10. Ensures complete accurate and organized records in accordance with agency policy.
  11. Ensures coordination of services across disciplines and individuals while ensuring appropriate use of funding sources.
  12. Balances good clinical outcomes with positive financial outcomes for the agency.
  13. Follows up with patients doctors to obtain proper orders and resolve any outstanding issues.
  14. Works closely with the DON to ensure departmental responsibilities are met.
  15. Represents the agency in meetings regulatory surveys and committees when needed.
  16. Acts as the DON in their absence.

Team Communication & Coordination

  1. Reinforces changes and ensures staff compliance with organizational policies and procedures.
  2. Provides informal clinical education to field staff within their discipline.
  3. Maintains high visibility and availability as a supportive professional.
  4. Fosters an environment that encourages teamwork positive attitudes and high morale.
  5. Serves as a role model to staff through a commitment to outstanding customer service.
  6. Communicates regularly with supervisors peers and other staff.
  7. May participate in interviews for potential new hires.

Field Staff Supervision and Development

  1. Conducts reviews of field staff notes SOC ROC and Recertification Documentation.
  2. Occasionally conducts field supervisory visits for clinicians.
  3. Provides input on field staff annual performance reviews.
  4. Assists in assigning new hires to a preceptor for field orientation.
  5. Mentors team members in patient management techniques clinical skills and critical thinking skills.

Regulatory Compliance

  1. Understands and ensures compliance with Medicare and State Conditions of Participation.
  2. Maintains compliance with all applicable laws and regulations.

Productivity and Reimbursement

  1. Understands the interrelationship between clinical care and financial reimbursement for beneficiaries.
  2. Ensures compliance with documentation requirements related to reimbursement.
  3. Ensures adherence to payer requirements for reimbursement.
  4. Proactively reviews insurance authorizations to identify the need for extensions.
  5. Manages variances in productivity and identifies employee action plans to improve individual and departmental productivity.

Work Ethic and Commitment

  1. Understands and commits to MBI Health Services philosophy and policies and assumes clinical field responsibilities if needed.
  2. Meets deadlines occasionally requiring after-hours work.
  3. Recognizes that care management encompasses patient staff and regulatory commitments.
  4. Maintains referral relationships and takes necessary actionssuch as home visits apology letters or special callsto sustain them.
  5. Independently seeks opportunities within MBI Health Services to develop leadership and management skills in areas where they may identify weaknesses or growth opportunities.

Qualifications :

  • Must be a Registered Nurse (RN) in Washington DC. 
  • Bachelors Degree in Nursing 
  • Minimum of two (2) years of supervisory experience in a healthcare setting
  • Experience in case management patient coordination or clinical leadership preferred.
  • Home health experience is a plus.
  • Strong organizational computer and time management skills required.
  • Ability to work in a diverse environment.
  • Must pass a background check.

Additional Information :

Yearly Salary: $100000
