Position Summary

The organization is seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join its full-time primary care team in Bronx NY.

This is a full-time outpatient-focused role within a supportive multidisciplinary clinical environment. The Family Nurse Practitioner will deliver comprehensive primary care services to patients of all ages with a strong emphasis on preventive care wellness and management of acute and chronic conditions.

Essential Position Responsibilities

• Provide comprehensive primary care services including preventive care acute illness management and chronic disease treatment for patients of all ages

• Perform physical examinations; order interpret and evaluate diagnostic tests

• Develop and manage individualized treatment plans

• Educate and counsel patients on disease prevention health maintenance and lifestyle modification

• Collaborate closely with physicians specialists and clinical staff to ensure coordinated high-quality patient care

• Maintain accurate timely documentation in the electronic health record (EHR)

• Participate in care coordination quality improvement initiatives and population health efforts

Qualifications

• Education: Graduate of an accredited Nurse Practitioner program (MSN or DNP)

• Licensure & Certification: Active or eligible New York State Family Nurse Practitioner (FNP) license; national certification required

• Experience: Prior primary care or family medicine experience preferred; new graduates are welcome to apply
