Position Overview:

The IONM Technologist provides hands-on intraoperative neuromonitoring for Veterans undergoing complex surgical procedures including set-up acquisition and intraoperative troubleshooting of neurophysiologic data under the supervision of a remote Board-Certified Neurophysiologist. The role ensures real-time monitoring of spinal cord and nerve function to identify neural insult early and support timely surgical intervention in accordance with VA and national neuromonitoring standards.

• Work Schedule:
  • Core OR hours: typically 6:30 a.m. - 4:30 p.m. EST three scheduled IONM days per week (estimated) with approximately 5 hours per case.
  • 24/7/365 on-call coverage including evenings nights weekends and federal holidays; on-call technologist must be onsite within 60 minutes of call for emergent/unscheduled cases.

Qualifications

• Requirements:
  • Current CNIM (Certified Neurophysiologic Intraoperative Monitorist) certification from ABRET with ongoing compliance with CEU requirements.
  • Current unrestricted ability to work in a U.S. health care setting; must meet all citizenship/immigration and E-Verify requirements applicable to federal healthcare contracts.
  • Current BLS and ACLS certifications (or equivalent) prior to assignment.
  • Completion and maintenance of all VA-required training: Privacy and Information Security HIPAA radiation safety (fluoroscopy) active threat training and other mandatory education specified by BWCVAMC.
  • Must successfully complete VA credentialing processes (e.g. VetPro) and maintain current licensing/certifications at all times with documentation provided upon renewal.
  • Requires strict adherence to VA privacy HIPAA and infection prevention and information security rules with zero tolerance for breaches.
  • Position is part of a 24/7 coverage model with on-call and call-back responsibilities for emergent and after-hours cases.
• Experience:
  • At least 2 years of hands-on experience providing IONM during surgical procedures including demonstrated proficiency in performing neuromonitoring under physician supervision.
  • Compliance with VA infection control and occupational health requirements including negative TB testing or acceptable IGRA immunity/records for MMR Varicella Tdap annual influenza vaccine (or masking with medical exemption) and blood-borne pathogen training.
• Skills:
  • Advanced technical skill in IONM equipment setup troubleshooting and artifact recognition across multiple modalities (SSEP MEP EMG EEG).
  • Strong clinical judgment to distinguish clinically relevant signal changes from artifacts and escalate appropriately.
  • Excellent communication skills with surgeons anesthesiologists nurses and remote Neurophysiologists in a high-acuity OR environment.
  • Working knowledge of relevant neurosurgical spinal vascular cardiac orthopedic and ENT procedures requiring IONM and associated neuroanatomy.
  • Proficiency with electronic documentation in an EHR and adherence to VA privacy security and data standards.
  • Ability to work independently respond rapidly to on-call requests and maintain professional conduct under stress in an OR setting.

Job Responsibilities:

• Set up calibrate and operate IONM equipment for modalities such as SSEP MEP EMG and EEG during spinal neurosurgical vascular cardiac thoracic orthopedic ENT and related procedures.
• Perform continuous intraoperative neurophysiologic monitoring from induction of anesthesia until case completion documenting baseline and ongoing changes in signals.
• Communicate real-time neurophysiologic changes to the remote Neurophysiologist and surgical/anesthesia team following established escalation and reporting protocols.
• Prepare place and remove electrodes probes and other common supplies (needle electrodes gel electrodes cortical screw electrodes ground pads probes/tubes) in accordance with sterile technique and device safety requirements.
• Document all IONM data events and communications in the VA Electronic Health Record (EHR) in accordance with VHA Handbook 1907.01 and local documentation policies.
• Adhere to AANS ABRET Joint Commission and VA standards of care as well as BWCVAMC OR policies and procedures.
• Participate in quality assurance activities including timely incident reporting in the VA Patient Safety Reporting System and cooperation with QASP monitoring (direct observation random sampling credential reviews).
• Maintain equipment functionality coordinate with VA Biomedical Engineering for inspections and support vendor/biomed work as required before equipment is placed into service.
• Support billing documentation by maintaining accurate case records that align with contract line items and hours billed.

Physical Examination Responsibilities:

• Perform and document pre-procedure IONM-related assessments (e.g. electrode site assessment skin integrity positioning considerations) as directed by the supervising Neurophysiologist and surgical team.
• Continuously monitor the functional integrity of neural structures via neurophysiologic modalities recognizing patterns indicative of potential injury and initiating escalation.
• Assist OR staff in safe positioning and protection of extremities and pressure points as it relates to optimal neuromonitoring signal acquisition.

Medical Oversight Responsibilities:

• Operate under the technical and clinical oversight of a remote Board-Certified Neurophysiologist and the VA Chief of Neurosurgery or delegate following their intraoperative directives.
• Immediately communicate significant changes in monitored signals technical limitations and artifact concerns to the supervising physician and surgical team.
• Support the supervising physician in post-case documentation and case quality review as requested.
