LP Therapy is seeking a licensed Speech-Language Pathologist (SLP) to provide evaluation and treatment of speech language cognitive and swallowing disorders in a Long-Term Care setting.
Responsibilities: review medical charts; evaluate speech language cognition and swallowing; develop and implement individualized treatment plans; modify programs based on patient progress; provide therapy for communication cognitive and swallowing deficits; complete timely and accurate Medicare and facility documentation; collaborate with interdisciplinary teams; support patient satisfaction and quality outcomes; comply with productivity and regulatory standards.
Qualifications: Current state SLP license (Masters degree with CCC required); LTC experience preferred; strong knowledge of Medicare documentation; excellent communication organization and time-management skills; ability to work independently and as part of a team.
Work Environment: Long-term care facility; physical demands consistent with therapy services.
