Job Summary

We are working closely alongside with a local authority in Liverpool to assist with the appointment of an Outreach Family Support Worker on a 1-months contract highly likely to be extended at clients discretion. Please apply with your CV for immediate consideration.

Rate of Pay: 16.50- 21.70 per hour

Summary:

The Outreach Family Support Worker provides high-quality home-based Early Help support to children (ages 0-25*) and their families using a whole family approach. The primary goal of this role is to deliver intensive targeted intervention at Level 3 of the safeguarding continuum to prevent the need for statutory social care services. The post-holder takes whole-case responsibility as a Lead Professional coordinating multi-agency support and utilizing the Signs of Safety (SOS) model to empower families and improve outcomes for vulnerable children.

Responsibilities:

  • Act as the Lead Professional for complex cases stepping down from Childrens Social Care. This includes completing Early Help Assessments (EHAT) chairing Team Around the Family (TAF) meetings and formulating intervention plans.
  • Deliver bespoke packages and programmes for vulnerable children and adolescents including those with complex emotional and mental health challenges.
  • Support parents and carers in developing practical strategies to enhance positive relationships and improve home environments.
  • Assess risk through consistent home visits identify when cases escalate to Level 4 (Statutory Care) and ensure a smooth transition while promoting the safeguarding of children in line with statutory legislation.
  • Support the delivery of supervised contact for cases open to Early Help as required.

Qualification:

  • Up-to-date knowledge of the legal framework and statutory responsibilities regarding the protection and care of children.

Essentials:

  • Significant experience working with children young people and families within an outreach or home-based setting.
  • Experience taking lead responsibility for cases including the coordination of multi-agency meetings (TAF/LP roles).
  • Proven track record of assessing risk and need using standardized assessment tools (e.g. EHAT).
  • Experience supporting adolescents with mental health issues or families with complex emotional needs.
  • Experience using de-escalation techniques to manage challenging behaviors in a community or home environment.

  • This role requires an Enhanced DBS
