Position Summary
The Virtual Care Coordinator plays a key role in supporting virtual patient care programs within a large healthcare system. This position focuses on remote patient monitoring virtual observation and telehealth coordination to enhance patient safety clinical quality and operational efficiency. The coordinator works closely with clinical teams leadership and technology partners to ensure effective telehealth workflows and high-quality patient-centered care.
Key Responsibilities
Support virtual patient monitoring observation and telehealth services.
Perform quality assurance activities peer reviews and program evaluations; recommend process improvements.
Participate in case reviews and collaborate with hospital partners to implement and track outcomes.
Triage and assign patients for virtual monitoring and observation services.
Maintain operational databases and track performance metrics response times and utilization.
Identify trends and support corrective action initiatives.
Provide real-time technical and workflow support to clinicians using telehealth platforms.
Monitor early warning indicators and escalate patient safety or clinical concerns promptly.
Ensure accurate documentation of patient behaviors interventions and escalations.
Monitor telehealth consult orders and scheduled virtual visits to ensure timely execution.
Train and onboard new staff on virtual care workflows systems and documentation standards.
Serve as a liaison between clinical teams technology partners and telehealth vendors.
Maintain patient privacy and comply with HIPAA and information security policies.
Submit shift reports and participate in staff meetings and team briefings.
Qualifications
Minimum of 3 years of direct patient care experience including telehealth or remote monitoring.
Experience with telehealth workflows quality improvement and outcomes measurement.
Ability to identify and escalate changes in patient condition or behavior.
Strong collaboration skills with interdisciplinary healthcare teams.
Proficiency with electronic medical records (Epic preferred).
Ability to analyze and apply telehealth-related policies and procedures.
Strong communication critical thinking prioritization and problem-solving skills.
Comfort navigating multiple technology platforms and supporting clinicians in real time.
Sensitivity to language cultural and accessibility considerations in virtual care.
Associates or Bachelors degree in a health-related field (preferred).
