Full-Time Day/Evening Rotation (5x8s)
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated nursing team. This is an excellent opportunity for an LPN who thrives in a collaborative resident-centered environment and is passionate about delivering high-quality care to adult and geriatric populations.
Schedule & Commitment
Full-Time Day/Evening Rotation (5x8-hour shifts)
Every Other Weekend (EOW) & Holidays Required
Requirements
Active NYS LPN License
BLS Certification (American Heart Association or RQI only)
Minimum 1 year experience as an LPN
LTC (Long-Term Care) experience required
Ability to work with adult and geriatric residents
Position Overview
The LPN demonstrates the knowledge and clinical skills necessary to provide age-appropriate culturally competent nursing care under the supervision of a Registered Nurse. This role supports residents physical and psychosocial needs while contributing to a safe therapeutic environment.
Key Responsibilities
Provide direct and indirect skilled nursing care to assigned residents
Collect and communicate clinical data related to residents physical and psychosocial status
Collaborate with RNs and the interdisciplinary care team to develop and update resident care plans
Assist in implementing individualized age-appropriate plans of care
Evaluate resident outcomes and recommend care plan revisions or referrals as needed
Promote a safe healthy and supportive environment for residents and families
Maintain accurate documentation and compliance with facility policies
Participate in staff orientation mentoring and ongoing education
Demonstrate leadership and professionalism within the scope of LPN practice
Professional Expectations
Maintain current clinical knowledge and competencies
Apply safety cost-awareness and quality considerations in care delivery
Work effectively with physicians nursing staff and ancillary departments
Understand and comply with infection control and bloodborne pathogen exposure standards
