Full-Time Day/Evening Rotation (5x8s)

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated nursing team. This is an excellent opportunity for an LPN who thrives in a collaborative resident-centered environment and is passionate about delivering high-quality care to adult and geriatric populations.

Schedule & Commitment

  • Full-Time Day/Evening Rotation (5x8-hour shifts)

  • Every Other Weekend (EOW) & Holidays Required

Requirements

  • Active NYS LPN License

  • BLS Certification (American Heart Association or RQI only)

  • Minimum 1 year experience as an LPN

  • LTC (Long-Term Care) experience required

  • Ability to work with adult and geriatric residents

Position Overview

The LPN demonstrates the knowledge and clinical skills necessary to provide age-appropriate culturally competent nursing care under the supervision of a Registered Nurse. This role supports residents physical and psychosocial needs while contributing to a safe therapeutic environment.

Key Responsibilities

  • Provide direct and indirect skilled nursing care to assigned residents

  • Collect and communicate clinical data related to residents physical and psychosocial status

  • Collaborate with RNs and the interdisciplinary care team to develop and update resident care plans

  • Assist in implementing individualized age-appropriate plans of care

  • Evaluate resident outcomes and recommend care plan revisions or referrals as needed

  • Promote a safe healthy and supportive environment for residents and families

  • Maintain accurate documentation and compliance with facility policies

  • Participate in staff orientation mentoring and ongoing education

  • Demonstrate leadership and professionalism within the scope of LPN practice

  • Maintain current clinical knowledge and competencies

  • Apply safety cost-awareness and quality considerations in care delivery

  • Work effectively with physicians nursing staff and ancillary departments

  • Understand and comply with infection control and bloodborne pathogen exposure standards

$300.00 Meals & Incidentals Per Diem (Tax Free)
$500.00 Lodging Per Diem (Tax Free)
