The Registered Nurse provides skilled nursing care case management and hospice care to individuals. Responsibilities include patient assessments medication administration treatment planning and interdisciplinary team collaboration. Strong communication and collaboration skills are essential for working with patients providers and team members.

Responsibilities:
- Provide skilled nursing care to patients.
- Conduct patient assessments and develop treatment plans.
- Administer medications and treatments as prescribed.
- Collaborate with the interdisciplinary team to ensure comprehensive patient care.
- Communicate effectively with patients providers and other team members.
- Case management.
- Hospice care.
Required Skills & Certifications:
- Active Registered Nurse license in the state of working.
- Clinical Assessment Medication Administration and Treatment Planning skills.
- Interdisciplinary Team Collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Bachelors degree in Nursing or related field.
- Minimum of 2 years of hospital home health or hospice care experience.
- Drivers license car insurance and automobile for field work.
- Stroke experience.
Preferred Skills & Certifications:
- Specialities in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room.
Special Considerations:
- On-call required must live within 30 minutes of the hospital for stroke call.
- May be required to float to other facilities.
Scheduling:
- 12 on-call shifts per 6-week period including one weekend.
