Are you passionate about making a difference in adult social care We are seeking dynamic and compassionate Team Leaders to join our Home First Central Duty and Hospital Team. This is an exciting opportunity to lead and inspire a team of Community Support Workers Customer Support Agents and Care Brokers ensuring the delivery of high-quality person-centred care that empowers service users to maximise their independence and achieve positive outcomes.


About the Role

The Hospital Discharge team works in partnership with the University Hospitals of Leicester Leicestershire Primary Care Trust and the Intergrated Care Board supporting the timely discharge of Leicestershire residents. The team work tirelessly to support people back to their own home with adequate care and support maximising their independence and recovery. For those people who are unable to return home the team will ensure people continue their recovery in the most appropriate residential or nursing setting.

The Community Duty team manages all the incoming referrals into Home First. Identifying and managing the immediate risk to adults with physical and mental health whilst maximising their independence and meeting their desired outcomes. The team works in partnership with colleagues both internal and external to ensure people receive the right level of care and support from the most appropriate agency.

Both teams work at pace with high volumes and are required to be dynamic and flexible to meet the needs of the community whilst striving to meet their personal KPIs. The role of the Team Leader is key to the success of the service coordinating the day-to-day work performance managing their staff whilst building and maintaining processes that are lean and robust.

The team leaders are responsible for leading and supporting the team to ensure effective and integrated working practices. This includes promoting personalisation choice and self-directed support while safeguarding vulnerable adults in line with statutory and council policies. The team leaders manage performance resources and budgets to maintain high standards and drive continuous improvement. Collaboration with key partners such as the NHS housing voluntary and independent sectors and other council departments is essential. Additionally they will contribute to service development staff training and the implementation of best practice.

The key duties:

  • To improve outcomes for people by ensuring that staff can provide good outcomes for people by implementing the departments operating model and ways of working to deliver social care
  • To provide technical and professional support to team members.
  • To improve ways of working for staff by supporting and implementing improved working between team childrens services and partner agencies with an MDT focus.
  • To manage staffing budgets and staff performance using departmental and corporate policies ensuring effective and efficient team capacity capability and delivery.
  • To ensure the effective management of team performance and resources.
  • To provide performance and delivery reports to Strategic Service Managers and HOS

About You

The successful applicants will demonstrate that they can perform the positions main duties and responsibilities at both application and interview. Be educated to degree level or equivalent management experience

  • A degree or equivalent management experience or a professional qualification in social work or occupational therapy.
  • Significant experience in adult social care including people and performance management.
  • Strong analytical organisational and communication skills.
  • Experience of partnership and multi-agency working.
  • Use a range of information to provide clear decision making and prioritise and review performance against targets set
  • Experience of supervising staff setting clear priorities and objectives and negotiating and implementing effective outcomes
  • A commitment to equality diversity and safeguarding.
