Primary Job Responsibilities:
- Conduct comprehensive patient assessments to evaluate their healthcare needs.
- Develop implement and update individualized care plans based on patient assessments and preferences.
- Administer medications and treatments as prescribed by the physician.
- Monitor and document patient progress and responses to treatment.
- Educate patients and their families on health management medication administration and care plans.
- Coordinate care with interdisciplinary team members to promote optimal health outcomes.
- Ensure compliance with agency policies state and federal regulations.
- Act as a patient advocate promoting their rights and preferences in care.
Requirements - Current RN license in good standing to practice in the Commonwealth of Pennsylvania.
- Graduate of an accredited nursing school (BSN preferred).
- At least 1 year of nursing experience; experience in home health preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent interpersonal and communication skills.
- Ability to work independently and manage time effectively.
- Reliable transportation and valid Drivers License.
- Proof of COVID-19 Vaccination or an approved Medical or Religious exemption.
- Proof of Annual Influenza vaccination or an approved Medical or Religious exemption.
