Primary Job Responsibilities:

• Conduct comprehensive patient assessments to evaluate their healthcare needs.
• Develop implement and update individualized care plans based on patient assessments and preferences.
• Administer medications and treatments as prescribed by the physician.
• Monitor and document patient progress and responses to treatment.
• Educate patients and their families on health management medication administration and care plans.
• Coordinate care with interdisciplinary team members to promote optimal health outcomes.
• Ensure compliance with agency policies state and federal regulations.
• Act as a patient advocate promoting their rights and preferences in care.

• Current RN license in good standing to practice in the Commonwealth of Pennsylvania.
• Graduate of an accredited nursing school (BSN preferred).
• At least 1 year of nursing experience; experience in home health preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent interpersonal and communication skills.
• Ability to work independently and manage time effectively.
• Reliable transportation and valid Drivers License.
• Proof of COVID-19 Vaccination or an approved Medical or Religious exemption.
• Proof of Annual Influenza vaccination or an approved Medical or Religious exemption.
