Our client is seeking an experienced Band 6 Occupational Therapist to join their team in Barrow-in-Furness. This role focuses on discharge planning ensuring safe and timely transitions from hospital to home or community settings and working closely with patients and the multidisciplinary team.
Key Responsibilities:
- Lead and coordinate discharge planning for patients within an acute setting.
- Complete functional assessments and recommend equipment or adaptations to support safe discharge.
- Work collaboratively with the multidisciplinary team to facilitate smooth transitions.
- Promote independence and optimize patient outcomes through evidence-based interventions.
Requirements:
- HCPC registration as an Occupational Therapist.
- Proven experience in discharge planning within acute or community settings.
- Strong understanding of discharge pathways and admission avoidance strategies.
- Excellent communication and teamwork skills.
- A valid driving licence and access to a car (desirable for community follow-up).
