Job Summary

Band 6 Occupational Therapist Discharge Planning

Location: Barrow-in-Furness
Contract: Temporary

Our client is seeking an experienced Band 6 Occupational Therapist to join their team in Barrow-in-Furness. This role focuses on discharge planning ensuring safe and timely transitions from hospital to home or community settings and working closely with patients and the multidisciplinary team.

Key Responsibilities:

  • Lead and coordinate discharge planning for patients within an acute setting.
  • Complete functional assessments and recommend equipment or adaptations to support safe discharge.
  • Work collaboratively with the multidisciplinary team to facilitate smooth transitions.
  • Promote independence and optimize patient outcomes through evidence-based interventions.

Requirements:

  • HCPC registration as an Occupational Therapist.
  • Proven experience in discharge planning within acute or community settings.
  • Strong understanding of discharge pathways and admission avoidance strategies.
  • Excellent communication and teamwork skills.
  • A valid driving licence and access to a car (desirable for community follow-up).
