Our client is seeking an experienced Band 6 Occupational Therapist to join their Community Stroke Service in Leeds. This role involves providing specialist occupational therapy interventions to patients recovering from stroke in the community supporting rehabilitation and promoting independence.
Key Responsibilities:
- Deliver high-quality occupational therapy interventions for stroke patients in a community setting.
- Complete functional assessments and develop individualized rehabilitation plans.
- Support discharge planning and liaise with the multidisciplinary team.
- Promote independence and optimize patient outcomes through evidence-based practice.
Requirements:
- HCPC registration as an Occupational Therapist.
- Proven experience in stroke rehabilitation ideally in a community setting.
- Strong communication and teamwork skills.
- Ability to work autonomously and manage a caseload effectively.
- A valid driving licence and access to a car (essential for community visits).
