Job Summary
Navitas Healthcare LLC is seeking a Licensed Clinical Social Worker (LCSW) to provide comprehensive behavioral health services in a clinical care setting. This position involves conducting assessments developing treatment plans providing therapeutic interventions and coordinating patient care from admission through discharge. The LCSW will work closely with psychiatrists nurses and multidisciplinary teams to ensure high-quality behavioral health services across inpatient outpatient and emergency department environments.
Responsibilities
Conduct clinical and psychosocial assessments for patients with acute behavioral health needs
Develop document and implement individualized treatment plans
Lead treatment planning and discharge coordination in collaboration with multidisciplinary teams
Provide individual family and group therapy services
Coordinate care to ensure continuity and safe transitions to community-based services
Assess ongoing psychosocial needs and arrange appropriate home care and community resources
Connect patients and families with hospital programs and external support services
Assist with placement decisions and referrals to appropriate facilities when needed
Provide crisis intervention and prevention services as required
Serve as a liaison between patients families healthcare providers and community agencies
Communicate with managed care organizations to obtain authorization for services
Maintain compliance with applicable regulatory and documentation standards
Provide coverage support for inpatient and partial hospitalization behavioral health services
Participate in community outreach and educational activities
Deliver professional compassionate service to patients families and colleagues
Perform additional duties as assigned
Qualifications
Active Massachusetts Licensed Clinical Social Worker (LCSW) license Required
Masters degree in Social Work (MSW) from an accredited program
Crisis Prevention Intervention (CPI) Certification Required
Basic Life Support (BLS) Certification Required
Ability to read write and communicate effectively in English
Experience in behavioral health inpatient emergency department or partial hospitalization settings preferred
