Job Summary
Transition Home Healthcare is an elite home health agency serving Houston TX and surrounding areas since 2010. Occupational Therapist (OT)
Shifts Available: Full Time
Service Areas: Humble Atascocita Kingwood Crosby Highlands
Responsibilities:
- Facilitate rehabilitation of patients with emotional physical or mental disabilities.
- Develop and implement occupational therapy care plans.
- Coordinate care with home healthcare team.
- Evaluate therapy needs and monitor and report patient progress and outcomes.
- Educate patients and family members.
- Prepare timely complete and accurate clinical notes.
- Communicate effectively with patients caregivers healthcare providers office support and other care team members.
Qualifications:
- Occupational therapist in the state of Texas.
- Home health experience highly preferred.
- Knowledge of home health regulations and general healthcare compliance.
- Able to handle physical workload.
- Good driving record.
- Experience with HCHB EMR (preferred but not required).
- Effective communication and people skills.
