Title: Psychiatric Nurse Practitioner ACT Team
Location: Westchester County NY

Responsibilities of the Psychiatric Nurse Practitioner
Provide psychiatric assessments medication management and ongoing clinical oversight to adults enrolled in the ACT program.
Deliver community-based care meeting clients in their homes or other community settings as needed.
Collaborate with a multidisciplinary team including psychiatry social work nursing vocational specialists and peer support providers.
Participate in daily team meetings and treatment-planning sessions to ensure coordinated person-centered care.
Maintain accurate and timely clinical documentation in accordance with regulatory standards.
Contribute to crisis intervention risk assessment and development of safety plans when necessary.
Support recovery-oriented culturally responsive and trauma-informed approaches to care.
Requirements of the Psychiatric Nurse Practitioner
Active New York State Nurse Practitioner License (PsychiatricMental Health Nurse Practitioner certification required).
Masters or Doctoral degree from an accredited Nurse Practitioner program.
DEA registration or eligibility to obtain.
Experience working with individuals with serious mental illness; ACT Team or community-based psychiatric experience preferred.
Valid drivers license and ability to travel within Westchester County for community visits.
Strong communication organizational and interdisciplinary-collaboration skills.
