SUMMARY: The Native American Community Clinic (NACC) is a community health center in Minneapolis dedicated to serving urban Indigenous communities and other underserved groups. We are seeking a compassionate dedicated community-minded Parental Patient Advocate to join our team. This position plays a vital role in supporting parents and caregivers by promoting family unity and wellness coaching parenting skills facilitating classes coordinating parenting resources and providing support to families with CPS involvement. The Parental Patient Advocate works closely with clinical teams and community partners to reduce barriers learn critical parenting skills and improve family wholeness through culturally responsive holistic support.
DUTIES AND RESPONSIBILITIES:
- Provide direct support to patients in identifying and addressing social needs that impact health including transportation food insecurity insurance enrollment and other daily living concerns
- Provide in-clinic support to patients byidentifyingbarriers to completing medical dental and behavioral health treatment plans and developing solutions
- Collaborate with the healthcare team to ensure patients receive comprehensive integrated and culturallyappropriate care
- Maintainaccurateandtimelydocumentation of patient interactions services provided and outcomes
- Assistpatients in navigating public benefits including SNAP Housing Support and other relevant programs
- Provide MNsure navigation and outreach for uninsured/underinsured patients and community members
- Provide support and advocacy during interactions with CPS tribal child welfareor otheragencies
- Lead and coordinate parenting-focused classes and skills groups such as Circle of Support CPR/First Aid car seat safety andtrauma-informedparenting workshops
- Provide individualized coaching in the clinic and whenappropriate in the home to reinforce practical parenting strategies and promote confidence in caregiving
- Coordinate maternal child health meetings and contribute to multidisciplinary care planning
- Conduct in-reach and outreach activities to assess needs offer support build trust and connect families to services that improve health and long-term outcomes
- Build andmaintainpartnerships with community organizations offering parenting classes car seat programs child development resources early learning programs and culturalsupports
- Maintain up-to-date accessible resource directories related to parenting benefits child safety counseling traditional healing food and transportation support and other identified needs
- Provide support in grant-related supply ordering tracking and reporting
- Perform other duties as assigned

QUALIFICATIONS AND EDUCATION REQUIREMENTS:
- 2yearsof experience working to support families in a case management social service or clinical capacity
- Active Minnesota Drivers License and clean driving record
- Knowledge of local and/or tribal community resources and public benefits programs
- Familiarity with health-related social needs and social determinants of health
- Ability to work independently and collaboratively across interdisciplinary teams
- Excellent communication interpersonal organizational and problem-solving skills
- Proficiency with electronic health records and other relevant software (Ex. Epic Microsoft Office Suite etc.)
- Associates degree from a 2-year college or technical school or equivalent combination of education and experience
- Strong commitment to health equity and serving diverse underserved populations
- Experience working with Indigenous communities or tribal entities preferred

- Occasionally work near moving mechanical part
- Occasionally work in high precarious places
- Occasionally work around fumes airborne particles or toxic chemicals
- Occasional exposure to outside weather conditions
- Frequently exposure to bloodborne and airborne pathogens or infectious materials
- While performing the duties of this job the noise level in the work environment is usually moderate to loud
