Job Summary

Position Summary
The Parental Patient Advocate provides care coordination advocacy and practical support to pregnant and parenting patients. This role works closely with OB and other care teams to reduce barriers to care strengthen family stability and improve health outcomes using trauma-informed and harm-reduction approaches.

Core Responsibilities

  • Care Coordination & Navigation
    • Coordinate care for pregnant and parenting patients across medical behavioral health and social service systems
    • Identify and address barriers to care such as transportation housing instability food insecurity insurance gaps substance use and safety concerns

  • Advocacy & Systems Support
    • Assist patients with public benefits navigation including MNsure SNAP and Housing Support
    • Provide advocacy and support related to CPS tribal child welfare and other systems as appropriate
    • Participate in maternal-child health meetings and multidisciplinary care planning

  • Trauma-Informed & Harm Reduction Support
    • Provide patient-centered support grounded in trauma-informed and harm-reduction frameworks
    • Collaborate with OB behavioral health and harm reduction teams to support patients impacted by substance use and complex trauma
  • Parenting Support & Community Engagement
    • Coordinate parenting education and skill-building opportunities
    • Engage with community partners to connect families to culturally responsive parenting early childhood and family support resources

  • Outreach & Documentation
    • Conduct in-reach and outreach to build trust and connect families to resources
    • Maintain timely and accurate documentation
    • Perform other duties as assigned

Strong communication and documentation skills are essential in this fast-paced clinical environment.
