LPN Home Health Full-Time Day Shift

Warm Hearth at Home isseeking a compassionate and skilled Licensed Practical Nurse (LPN) tojoin our Home Health team on a FT-PT basis. The LPNwill deliver high-quality nursing care to patients in their homes workingunder the supervision of a Registered Nurse and in accordance with the plan ofcare state licensure guidelines and organizational standards.

Were looking for someone with strong critical thinking skills a hospital or home health background and excellent assessment skills. Someone with strong communication skills the ability to time management and someone that adapts well to change.

Responsibilities:

• Provide skilled nursing care to patients in their homes as directed by the RN and physician orders
• Perform clinical procedures such as wound care catheter care medication administration and monitoring vital signs
• Document timely accurate and complete patient information in the electronic medical record
• Collaborate with interdisciplinary team members to ensure coordinated care and positive outcomes
• Communicate effectively with patients families and caregivers to educate and provide support
• Recognize and report changes in patient condition to the RN or physician promptly
• Maintain compliance with all policies procedures and regulatory requirements
• Assist with obtaining and tracking authorizations for services working with the clinical and administrative teams

Qualifications:

• Current LPN license in the state of VA
• Minimum of one (1) year of LPN experience; home health experience preferred
• Valid drivers license reliable transportation and proof of auto insurance
• CPR certification (BLS) required
• Excellent communication clinical and organizational skills
• Ability to work independently and manage time effectively
