What Youll Do in This Role:
- The Physician Assistant (PA) works collaboratively with LifeCircles medical providers and the interdisciplinary team (IDT) to deliver high-quality participant-centered care. Acting within the scope of their practice the PA provides medical care to LifeCircles participants across multiple settings including day centers home visits hospitals nursing homes and assisted living facilities ensuring comprehensive support and continuity of care.
Primary Responsibilities:
Patient Care & Assessment:
- Assess initiate and coordinate participant care in collaboration with the medical team interdisciplinary team (IDT) and external agencies.
- Monitor chronic conditions address acute health concerns and provide treatment in the clinic participants homes or care facilities (ALF SNF AFC).
- Facilitate advanced care planning discussions with participants and caregivers.
Education & Family Engagement:
- Educate participants and families on managing chronic conditions and treatment plans.
- Communicate effectively with families and caregivers to support informed decision-making.
Interdisciplinary Team Collaboration:
- Participate in daily IDT meetings and contribute to collaborative care planning.
- Coordinate with external partners to support smooth transitions and access to services.
Quality Improvement & Compliance:
- Support quality improvement initiatives and ensure compliance with organizational and regulatory standards.
On-Call & After-Hours Support:
- Provide rotating on-call and weekend support as needed to address urgent care needs.
What Youll Need:
Education & Certification:
- Completion of an accredited Physician Assistant program
- Current unrestricted Physician Assistant license in Michigan
- Basic Life Support (BLS) and CPR certification (or willingness to obtain)
- Current DEA registration and Michigan Controlled Substance License
Experience:
- Minimum of 1 year of experience with frail elderly or long-term care populations preferred
