Description:

Deliver patient-centered outcome-driven pharmaceutical care for individuals across all age groups. Ensuretimelyandaccuratemedicationdistribution andprovide reliable drug information to patients and healthcare professionals to support safe andeffective therapy.

ESSENTIAL FUNCTIONS

  • Knows understands incorporates anddemonstratesthe Trinity Health Mission Vision and Values in behaviors practices and decisions.
  • Provide comprehensive pharmaceutical care including interpreting and verifying medication orders assessingclinical appropriatenessandmonitoringpatientsmedication profilesfor allergiescontradictionsduplicate therapiesinteractions and adverse effects whilemaintainingaccuratepatient profiles.
  • Collaborate with physicians nursesand other healthcare professionals to clarify orders recommend therapy adjustments and answer drug information inquiries.
  • Educate patients and families on medication use adherence and safety; conduct Medication Therapy Management (MTM)interviewsand document interventions.
  • Accurately prepare compoundanddispensemedicationsentering orders into the pharmacy system with proper documentationmaintainingrecords of controlled substance disbursementsand monitoringcompliance with dosing guidelines and drug protocolsintervening when necessary.
  • Administer vaccines (after certification) and ensure documentation in IRIS;comply withall state and federal pharmacy laws and regulations.
  • Provide oversight and accountability for pharmacy technician activities includingverifying medication orders they prepare andreviewing audit results to ensure accuracy safetyand compliance.
  • Ensure adequate inventory of pharmaceuticals and supplies andperformunit inspections in assigned areas tomaintaincompliance and operational efficiency.
  • Accurately bill prescription claims to third-party payers and manage order compilation and invoicing for provider offices and EMS services.
  • Participatein specialty pharmacy care management and quality improvement activities tooptimizepatient careand operationalperformance.
  • Serve as a preceptor for new colleaguesand support experiential education for pharmacy students.
  • Other duties as needed and assigned by the manager.
  • Maintains a working knowledge of applicable Federal State and local laws and regulations Trinity Healths Organizational Integrity Program Standards of Conduct as well as other policies and proceduresin order toensure adherence in a manner that reflects honest ethical and professional behavior.

MINIMUM QUALIFICATIONS

  • MustpossessaBachelor of Science degree inPharmacy or PharmDfrom an ACPE-approved college of pharmacy.
  • Must bea licensed pharmacist in the State of Iowaor eligible forlicensure in the State of Iowa. Pharmacistlicense must be obtained withinthree(3)months of hire.
  • MustpossessaPharmacy-Based ImmunizationDeliverycertificate ofcompletion through the American Pharmacists Association (APhA) and qualifyfor the ability to order and administer vaccinations in the State of Iowaper Iowa Code.
  • Successful completion of an ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years of ambulatory pharmacy preferred.
  • Basic Life Supportcertified withinthree (3)months of hire.
  • Completionof the Iowa Department of Health and Human Services MandatoryReporterChild and Dependent Adult Abuse training within three (3) months of hire.
  • Must be comfortable operating in a collaborative shared leadership environment.
