Description

The Supv Adult Care Asst LPN provides Adult Care Assistant Supervision and ADHC recordkeeping along with some nursing care to participants under the direct and indirect supervision of the Center Manager or designee. The position requires good interpersonal skills and the ability to communicate effectively at all levels.



Responsibilities
  1. Supervision
    1. Makes sure that the job requirements of each Adult Care Assistant are met accordingly to standards. Assists in the coordinating and overseeing PACE Day Health Care Programs with required recordkeeping.
    2. Responsible for the coordination with Adult Care Assistants of Basic ADL Care of designated participants. This includes but not limited to bathing dressing catheter care basic skin care and incontinent care toileting processes and meals and snacks.
    3. Responsible for ensuring the Adult Care Assistants actively participate as assigned with the Recreational Therapist by scheduling assignments of activities in the different work areas provided.
  2. Leadership:
    1. Using nursing process delegates care to appropriate personnel; integrates cost effective measures into nursing practice; recognizes unit problems.
    2. Takes responsibility for the work schedule of the Adult Care Assistant including but not limited towork assignments such as: room assignments bathing assignments charting participant load assignments and recording absenteeism and tardiness and leaving early.
    3. Reports any identified needs for the multidisciplinary health care services and alternative levels of care to appropriate personnel in order to ensure optimal continuity and expediency throughout the health care delivery system.
  3. Quality
    1. Takes responsibility for documentation; demonstrates active participation in QAPI processes; complies with PACE CENTER expectations to meet staffing demands based on participant care needs. Complies with policies addressing safe working conditions; monitors unsafe working conditions and recognizes inappropriate participant care management resolves issue/problem and completes written reports.
    2. Participates in data collection toward assisting the Registered Nurse in assessing participant physical psychological and functional assessment. Able to provide input to the Registered nurse in problems that can result with the participants/clients of ADHC.
  4. Technical Tasks
    1. Documents treatment/procedures observations nursing care and participant activities in EMR as deemed necessary.
    2. Evaluations: responsible for the written evaluations of all Adult Care Assistants and presentation thereof.
    3. Responsible for ordering or designation thereof supplies
  5. Communication and Collaboration
    1. Implementing/Managing Care: Care provided conforms to accepted practice standards; provides correct treatments/procedures and other care as prescribed and according to patient care standards.
    2. Demonstrates understanding of age-related characteristics and needs of participants served; explains nursing procedures in appropriate Demonstrates and maintains clinical competency in the delivery of participant care services in accordance with the specific identified needs of PACE participants. Provides basic nursing care to assigned participants.
    3. Provides individualized appropriate and compassionate participant care in collaboration with team members utilizing a participant-specific plan with treatment and rehabilitative goals for desired outcomes.
  6. Other Duties as Assigned
    1. Performs other duties as assigned.
    2. Only act within the scope of his or her authority to practice


Qualifications
  • Must have one year nursing experience working with the frail/elderly population. Needs to have 1 year supervisory experience with staff members preferably with the C.N.A.
  • Graduate accredited LPN program
  • Moderate computer skills in Word and Excel
  • LPN current License in state of Louisiana; Current CPR certification BLS
