Job Summary

Post Acute Care Physician opportunity in Media PA

Great full-time (5 days per week) primary care role you will have the opportunity to manage complex patient cases in post-acute settings (skilled nursing and independent living facilities) in Media Pennsylvania focusing on improving care for some of the most vulnerable patients.

Primary care rounding in skilled nursing facilities- Flexible rounding schedule
Manage chronic disease diagnose illnesses and medical conditions and implement plan of care
Provide oversight guidance and mentorship to our nurse practitioners
Conduct clinical case reviews as needed and share tools information and guidelines as they relate to quality of care

Qualifications:

Minimum of 1 year of experience with a commitment to delivering high-quality patient-centered care
Post-acute care experience is beneficial though not required as we provide comprehensive support and development opportunities
Active and unrestricted Pennsylvania license (or in process)
Active and unrestricted DEA/prescriptive authority (or in process)
Internal medicine (IM) family medicine (FM) emergency medicine (EM) hospitalist or candidates with any related experience are welcome to apply

Compensation:

Compensation is fee for service (FFS). Expected compensation is an estimated range of $180000 to $291000 annually with no cap on productivity income potential.
