Post Acute Care Physician opportunity in Media PA
Great full-time (5 days per week) primary care role you will have the opportunity to manage complex patient cases in post-acute settings (skilled nursing and independent living facilities) in Media Pennsylvania focusing on improving care for some of the most vulnerable patients.

- Manage chronic disease diagnose illnesses and medical conditions and implement plan of care
- Provide oversight guidance and mentorship to our nurse practitioners
- Conduct clinical case reviews as needed and share tools information and guidelines as they relate to quality of care
Qualifications:- Minimum of 1 year of experience with a commitment to delivering high-quality patient-centered care
- Post-acute care experience is beneficial though not required as we provide comprehensive support and development opportunities
- Active and unrestricted Pennsylvania license (or in process)
- Active and unrestricted DEA/prescriptive authority (or in process)
- Internal medicine (IM) family medicine (FM) emergency medicine (EM) hospitalist or candidates with any related experience are welcome to apply
Compensation:- Compensation is fee for service (FFS). Expected compensation is an estimated range of $180000 to $291000 annually with no cap on productivity income potential.
