The Opportunity

We are seeking a dynamic and results-driven Care Coordinator to serve as the face of Access TeleCare in the market working in tandem with clients and Access TeleCare clinicians to drive high quality clinical outcomes client satisfaction and provider satisfaction. As an expert in technical systems for Behavioral Health the Care Coordinator also serves as a key liaison between clinical teams technology platforms and operational leadership. This Care Coordinator is responsible for the implementation optimization and support of digital health platforms electronic medical records (EMRs) used across behavioral health services ensuring that platforms align with clinical workflows regulatory standards and organizational goals to improve patient outcomes and provider efficiency.

What youll do

  • Collaborate with clinical leadership to develop and document client-specific clinical workflows monitor the quality of care delivered and promptly address any gaps or issues
  • Partner with providers and clients to ensure key quality metrics are being met (e.g. documentation timeliness)
  • Serve as primary point of contact for on-service providers
  • Partner with clinical leadership and clients to address provider pain points to drive provider satisfaction and retention
  • Serve as primary operations point person with clients
  • Collaborate with the client services team to ensure client goals and retention are met
  • Partner with Implementation team and other internal stakeholders to successfully launch new programs including by establishing workflows and setting up appropriate internal systems
  • Create and update facility schedules that achieve practices quality and provider satisfaction goals
  • Ensure sufficient staffing is in place to support vacation and holiday schedules
  • Serve as subject matter expert on core clinical software systems supporting clients providers and peers in utilization of CRM and TIQ
  • Contribute to cross-functional initiatives with clinical and tech components
  • Other duties as assigned

What youll bring

  • Bachelors degree in a relevant field
  • Field related certifications a plus
  • 1-2 years of experience in health care services case management or hospital administration
  • 2 years of experience in EMR training or clinical systems support
  • Excellent organizational and interpersonal skills
  • Excellent project management skills
  • Robust computer skills including Microsoft Office suite
  • Ability to multitask and prioritize tasks
  • Understanding of provider scheduling
  • Able to act decisively and maintain discretion
  • Familiarity with common EMRs used in behavioral health (e.g. Netsmart Credible Epic etc)
  • Strong communication and interpersonal skills with the ability to train diverse learner groups
  • Background in clinical practice especially within behavioral health
  • Ability to analyze workflows and suggest training or process improvements
  • Proven ability to thrive in a high growth fast-paced organization and 100% Remote based environment
  • Must be able to remain in a stationary position 50% of the time
