Registered Nurse - Mental Health Hospital in the Home (HITH) - Sponsorship Available

Location: Melbourne Victoria

Salary: $108000 - $120000 per annum Super

Employment Type: Full Time

About the Opportunity

Be a foundational member of a new and innovative Hospital in the Home (HITH) service model for Mental Health launching in early 2026 in Melbourne. This is an exciting opportunity to deliver acute recovery-oriented mental health care in the community providing an alternative to traditional inpatient admission.

As an experienced Registered Nurse (Mental Health) you will work autonomously and collaboratively within a multidisciplinary team to provide intensive high-quality evidence-based care directly to consumers in their homes and community settings. Your work will be crucial in supporting consumers through acute mental health crises promoting recovery and preventing relapse within their usual environment.

Key Responsibilities

  • Clinical Care & Case Management: Deliver advanced person-centred and recovery-oriented mental health nursing care in the home setting.

  • Comprehensive Assessment & Risk Management: Conduct comprehensive mental state and psychosocial assessments formulate clinical risk assessments and develop robust individualized crisis and relapse prevention plans in collaboration with the consumer and their family/carers.

  • Therapeutic Interventions: Provide a range of evidence-based therapeutic interventions including psychoeducation medication management and supportive counselling.

  • Physical Health Monitoring: Conduct physical health assessments and observations monitor the physical effects of mental illness and psychotropic medications and liaise with General Practitioners and other health providers.

  • Crisis Intervention: Provide timely and effective crisis intervention and support facilitating admissions to inpatient or residential settings only when clinically indicated and necessary.

  • Care Coordination: Actively contribute to seamless care coordination discharge planning and effective linkage with community supports and continuing care services.

  • Documentation & Compliance: Maintain accurate timely and contemporaneous clinical documentation in the electronic medical record system ensuring practice adheres to the Mental Health and Wellbeing Act 2022 and relevant clinical standards.

  • Team Collaboration: Work collaboratively with the multidisciplinary team (e.g. Psychiatrists Social Workers Occupational Therapists Peer Workers) to ensure holistic and coordinated care.

Selection Criteria

Essential Qualifications & Experience

  • Current registration as a Registered Nurse (Division 1) with the Australian Health Practitioner Regulation Agency (AHPRA).

  • Postgraduate qualifications in Mental Health Nursing (or equivalent recognised specialisation - UK Mental Health Nursing Qualification is sufficient).

  • Experience:

    • Domestic Candidates: Minimum of three (3) years full-time equivalent experience in acute community or crisis intervention mental health settings.

    • International Candidates: Minimum of four (4) years full-time equivalent experience in acute community or crisis intervention mental health Ireland and NZ experienced highly regarded.

  • Drivers License

  • Advanced Clinical Skills: Demonstrated advanced skills in mental health assessment comprehensive risk assessment and clinical risk management.

  • Therapeutic Engagement: Exceptional ability to establish and maintain goal-directed therapeutic relationships with consumers from diverse backgrounds including strong communication and de-escalation skills.

  • Autonomy & Teamwork: Proven ability to work safely and effectively both autonomously in the community setting and collaboratively as part of a multidisciplinary team.

  • Quality & Safety: Commitment to quality improvement clinical governance and continuous professional development.
