Job Responsibilities

Flexible Schedule
Co-ordinated routes to Limit Travel

Coordinates total patient care by conducting comprehensive health evaluation monitoring the clients condition promoting sound preventive practices coordinating services and educational and training activities.

Submits clinical notes progress notes and other clinical record forms outlining the services rendered as indicated.

Job Requirements

Registered Nurse in The State of Florida
1 Year of Institutional or Home Health Experience - Preferred

Start of Care (SOC)/OASIS: 2.5 points (most complex).
Evaluation/Reassessment: 1.5 points.
Discharge: 1.25 points.
Routine Visit/Follow-up: 1 point.
Weekly Goal: Often 30 points equating to roughly 5-6 visits daily depending on complexity and documentation time.
