Hospice Admission Nurse

Schedule: Full-Time Monday-Friday (11am - 8pm)

Job Overview:

As an admission nurse youll conduct comprehensive patient assessments facilitate seamless admission procedures and deliver compassionate care coordination ensuring the best support for hospice patients and their families.

Responsibilities:

  • Coordinates and oversees all aspects of new patient admissions
  • Collaborates with patient care teams to ensure seamless communication
  • Educates families on patient condition needs and care
  • Performs head-to-toe physical assessments
  • Timely and accurately completes documentation
  • Other duties as assigned
Requirements

Education:Graduate of an accredited Diploma Associate or Baccalaureate School of Nursing.

Licensure:Current State license as a Registered Nurse; Current drivers license. Current CPR Certification.

Experience: Two (2) years experience as a Registered Nurse in a clinical care setting and one (1) year full-time hospice.

Skills: Nursing skills as defined as generally accepted standards of practice. Knowledge of state and federal regulations. Good interpersonal skills.

Transportation:Reliable transportation and valid auto liability insurance.
