POSITION SUMMARY

The Advanced Practice Provider (APP)/ Street Medicine is a Nurse Practitioner (NP) or Physician Assistant (PA) primarily responsible for working collaboratively with other health professionals within the Street Medicine program in providing high quality medical care to our unhoused patients as a part of a team-based model. They will provide exceptional patient care and will be skilled in networking and collaboration. This position will work with contracted Street Medicine vendors the MLK Community Health Foundation and national and international entities for MLKCH Street Medicine as programmatic needs arise.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Participates in strategic planning within overall needs in function of growth and clinical enterprise of MLKCH Street Medicine
  2. Facilitates relationship with other practitioners and learners
  3. Develops and educates learners
  4. Provides input for recruitment onboarding and education of new staff
  5. On-boards new clinical provider hires
  6. Provides input to the Manager of Population Health and Street Medicine for performance appraisals for staff
  7. Provides ongoing education to staff learners and community on health and homelessness
  8. Participates in development of new protocols for street medicine along with the Manager of Population Health and Street Medicine and medical director
  9. Participates in the development of standards for clinical application of new or established guidelines for patients
  10. Participates in local and state street medicine organizations (national and international optional)
  11. Participates in the development of quality standards for clinical operation in Inpatient and Outpatient setting along with the Manager of Population Health and Street Medicine and medical director
  12. Oversees quality of patient care with input from the clinical program leader and medical director
  13. Ensures accurate tracking and dispensing of pharmaceuticals
  14. Responsible for ensuring timely and accurate documentation
  15. Responsible for determining appropriate scope of practice with input from Manager of Population Health and Street Medicine and Medical Director
  16. Responsible for implementation of quality performance activities along with Manager of Population Health and Street Medicine
  17. Working collaboratively with Manager of Population Health and Street Medicine facilitates team perspective for staff at MLKCH MLKCHF and in the community
  18. Assists in development of MLKCH Policies and Procedures with input from Manager of Population Health and Street Medicine and medical director.
  19. Perform other duties as assigned or requested.

POSITION REQUIREMENTS


  • Graduate from an accredited Nurse practitioner program approved by the California Board ofNursing (MSN and/or DNP) or Graduate from an ARC-PA accredited physician assistant program
  • California State Medical License DEA
  • National Provider Identifier (NPI)
  • If Nurse practitioner must have current certification via American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP). Current and without restriction RN and NP license required.
  • If Physician assistant must have current certification by National Commission on Certification of Physician Assistants (NCCPA) and current and without restriction Physician Assistant license.

B. Qualifications/Experience

  • Current Basic Life Support (BLS) for Health Care Providers from the American Heart Association
  • Current Advanced Cardiac Life Support (ACLS) for Health Care Providers from the American Heart Association
  • Minimum experience is 1-year experience as an APP or at least 3 years of RN experience preferably in acute care setting.
  • 3-5 years of clinical experience as APP in related field (preferred)
  • Must complete annual Workplace Violence Prevention Program/Certificate per hospital policy during initial training/orientation but not to exceed 30 days from hire/transfer.
  • Valid unrestricted CA Drivers License

C. Special Skills/Knowledge

  • Bilingual skills preferred (Spanish)
  • Basic computer skills
  • Demonstrated ability to interact effectively with people at all levels of the organization
  • Ability to demonstrate knowledge of both private and public health care systems preferred
  • Demonstrated ability to utilize written and verbal communication presentation negotiation public speaking influence and collaboration skills with both management teams and multidisciplinary groups
  • Ability to promote and maintain patient health by providing medical services in collaboration with their supervising physician
  • Ability to obtain detailed and accurate medical histories and performs appropriate physical examinations on patients across the lifespan.
  • Ability to develop patient centered care plans which incorporate counseling on healthy lifestyle choices preventative medical care and psychosocial stressors as well as coordination of referrals to specialty and behavioral health services.
  • Ability to perform and/or interpret common laboratory radiological and other routine diagnostic procedures- as needed.
  • Ability to perform medical procedures/exams such as pap smears wound care suturing joint injections administration of immunizations or injections and infection management based on current privileges.
  • Ability to work collaboratively on a physician team to care for medically complex patients.
  • Ability to maintain accurate and up to date medical records and billing.
  • Ability to maintain safe and clean working environment by complying with clinic and health care system defined policies procedures and regulations
  • Demonstrated ability to protect patients and employees by adhering to infection-control policies and protocols.
  • Ability to participate in on-call for ill or injured patients and those seeking medical advice after hours
  • Ability to demonstrate sensitivity and awareness regarding the emotional cultural and socioeconomic aspects of the unhoused patient the patients condition and the patients family and social area
  • Lead and assist in education of health professional students
  • Develop health care team staff by providing information educational and experimental growth opportunities
  • Acknowledge compliance with federal state and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; and enforcing adherence to requirements and advising management on needed actions.
  • Ability to maintain professional and technical knowledge by attending educational workshops reviewing professional publications establishing personal networks and participating in professional societies.
  • Ability to lead and participate in Continuous Quality Improvement projects
  • Ability to maintain compliance with professional licensing requirements
  • Participate in activities to improve patient satisfaction
  • Participate in committees or workgroups as requested
