Physiotherapist Team Leader

Your Opportunity:

As a Team Leader at Plena Healthcare you will provide leadership and support across a dedicated region guiding operational and clinical services while fostering strong positive relationships with residents families and internal stakeholders. You will be part of a collaborative multidisciplinary team delivering personalised high-quality care across Residential Aged Care and Community settings.

In this role you will oversee the delivery of evidence-based mobility and functional programs care plan reviews one-to-one therapy and group-based active aging services ensuring your team provides efficient person-centred care that enhances the quality of life for every resident.

Duties will include but not limited to:

• Provide leadership and guidance to Allied Health teams to ensure high-quality person-centred care.
• Oversee assessments and clinical recommendations for mobility transfers falls prevention equipment needs skin integrity pain and joint management.
• Ensure best practice in care planning documentation and adherence to professional standards.
• Support team development through coaching feedback and performance guidance.
• Adapt to the evolving needs of Plena and the aged care facilities.

• Current AHPRA registration
• Australian working rights
• Willing to obtain NDIS workers screening and National Police Check
